I'd take Bio121. It's tougher, but you should have the background to do well so long as you work hard.
Spanish has classes called "Spanish for Medical Professions" or something like that. While it probably won't help you get into medical school too much, knowing Spanish, and having a little exposure to medical situations with Spanish, could help later on when applying for residency.
