Preview 3A Forms

When I previewed my application, the 3A form regarding the science supplement (nor any of the other supplements such as PLME) did not show up. I submitted my application online. Does this mean that my form 3A Science Supplement was also submitted?

Might this be because I chose Mathematics as my major in the application? However, I stated I wanted to study math in conjunction with physics, so I wrote the statement anyway. I just sent them an email containing my science statement to be safe.

I think that the instructions are to only fill out Form 3A if your primary interest is science.

It says: "This form is required if your primary interest is a Sc.B. or A.B. degree in Chemistry, Computer Science, Engineering or Physics." It sounds like you chose math. Then again, the way it is phrased makes it seem as though it's optional for non-science majors.

but why do they have extra questions for sciences and not for others..?

I put down biochem for my major, and I decided to do all the form 3A essays, and when I previewed it, they did not show up. I wrote to tech support, and they said

The PDF version of your application will not display some of your answers correctly. As long as they are saved to your application properly, they will be saved in the institution's database properly, even if they are not displaying correctly (or at all) on the PDF. You may want to check the pages of your application carefully before you submit it to ensure that your answers were saved properly.
I still wasn't very convinced (because it does say that the PDF preview is exactly what the admissions board is going to see!), but I just submitted it (ED).
