Is it necessary to use the Common App Recommendation Forms

My counselor told me that I had to give my teachers the Common App Teacher Recommendation form in order to get teacher recommendations. I forgot to give this to my teachers when I asked for recommendations, so now I have just written letters with none of the "evaluation" stuff that is on the Common App form. Would these still work?
