ED Agreement Form?

If we are applying ED to a college, do we print and mail the ED Agreement Form to the school?
I mean, is it mailed with the teacher recommendations / counselor recommendation?
I don't think so, but I just want to make sure. Haha.

I only realized after my school sent all the recs/forms out that I needed the ED form. So, I printed it off, signed it, had a parent sign it, brought it to the guidance office to have counselor sign it, and I mailed it to the college.

The ED form needs to be signed by student, parent and GC. So, after you and your parent have signed - bring it to GC. They will sign it and mail it out with the pkg that contains the transcript. Most schools have an ED form. If they use the common app, you can print out the ED form from the common app website. If they do not use common app, you will have to check the college's website to see if they do have an ED form for you to print out and sign. Some do not.
