failure??? please help

I need help with how to answer a short-answer question: Think of a recent failure AND achievement, how did they affect you, and so on. I can't think of either one. In terms of achievements, I get good grades and have good test scores, but they can see that on my application already. Same goes for my extracirriculars, no outstanding achievements there. As far as a recent failure, I'm totally clueless. I don't mean to sound arrogant, I know I'm not perfect, but I can't think of a failure that is significant enough to write something substantial about. I'd appreciate any suggestions.

really.. nothing.. you never didn't make the team, or didn't get an office you ran for, or didn't fit in with someone you wanted too, or made a B on a report you studied hard for, or failed to do what your parents wanted and felt bad about it, or lost your temper and later thought it would have been better not to do that?
Man you didn't experience the same high school I did....
here is my advice.. you need to realize that everything affects you.. otherwise we would all still be in diapers and unable to speak...
