I'm a senior looking to apply to grad school for math, and I'm beginning to round up some recommendations.

In one of my upper-level math courses, I currently have a 100, and the class average is around a 78. However, I never have any reason to go to office hours; the one time I did go, it was crowded with other students. So, I haven't been able to talk to my professor about my graduate school plans. I always participate in class, and haven't missed a day yet. I'm pretty sure he's aware of my name.

How would you suggest I try to get the recommendation? Should I just send him an email asking to set up an appointment to discuss graduate school plans, hinting at the fact that I want a recommendation? I DEFINITELY want a recommendation from this guy, since it's my only math class this semester.

Your plan sounds perfect.
Email as you indicated but don't ask about the rec until you see him in person. While you are their get his advice on programs, what you can do with a grad degree in math, etc... HE can probably offer you some good information, feel helpful, and not just be asked for the rec.
