It should be your app-id or your net id if you already got that and the password that you had during your application process.

what happens if we dont get our FA by may 1? all my things were sent in back in january, but they lost some papers and decided to notify me in april. arent they like legally obligated to notify me before may 1 or something?

Well I called them and they said that the FA decisions might not come by May 1st. So I asked them if I could ask over the phone about my FA and they said yes...told me to call back Tuesday (and this was after three phone calls, so call now)
