Do you use Duke's own teacher eval form (form B) or common app's?
I'm confused because it says you should use the Form B but if you are using common app shouldn't it be common app throughout?

Form B is a school supplement to the common app's school form. I used common app for everything, then got my guidance counselor to fill out form b and send it in too.

Okay so to clarify, my counselor needs to send in form B along with the common app secondary school report? In the same envelope?
