Two Quick, Perhaps Stupid Questions About the Supplement?

Under the 'Parent/Guardian Information' section, do I just leave the zip code and state blank if both of my parents were not born in the US?

And, also, what does the 'Legal Guardian' part mean, exactly? Do I only fill this part out if my parents are not also my legal guardians?

Yes, you do leave the state and zip code blank if your parents were born in another country.
No, you do not need to fill out that portion if your parents are your legal guardians.
