^I don't get it. I get that if you spell it backwards without the first 'T', you get the word 'hurt', but what does it mean?

^^^ It's the beauty of a Lupe Fiasco line. The obvious meaning is the play on the word truth. He's also saying that if you look at the truth in reverse (ie a lie) it could hurt.

It's one of my favorite double entendres from Lu (there's a ton).
