did any of you guys send in an extra letter of rec (besides GC, Science Teacher, Humanitarians Teacher)? If so, how did you do it without a form?

The head of the community service organization my son volunteered with wrote his own letter and just mailed it to the Admissions office last year. Admissions recommends including the student's name and birth date on any supplemental materials, so they can be sure to match it up with the correct applicant.
