Does MIT have an optional art supplement or optional reference form like Priceton and Stanford? If not, how/should I send them in? Just send the letter of recomendation by itself? If I send my CD in can I include my musical resume?

Send them anything with a note of explanation, or even call beforehand, and I think they will be happy to reveiw it. Perhaps more so in the case of a music sample, especially if it's something like tuba or trombone or chinese opera singing instead of piano or violin.

There's a place on the application to talk about any optional supplements you are sending [I believe it's on the last page of the online app, but don't take my word for it, I haven't looked at it since October ] I sent in a CD of my band along with a list of shows we have played and also the research paper I wrote for Intel STS. My guidance counselor included a resume and recommendation from the PI of my lab with all the school forms. So there's plenty of ways to send stuff. MIT likes to see varied kids so send what you think will help differentiate you.

That page doesn't say to send in anything, but it just says you can talk about anything you've created. I don't know how they deal specifically with supplements.

Oh my bad, (I whipped out my app just for you guys). Matt McGann (asst dir of admissions at MIT) says:
"What information should you put on any extra materials? Well, you should always put your name, and at least one (if not more) of the following data: date of birth, social security number, MIT ID #. As I said yesterday, the records office gets a large volume of mail, so you want to make sure that your materials are clearly marked so they get to your folder as soon as possible."
I assume that means supplemental materials are welcome

Actually, in http://blogs.mit.edu/madmatt/posts/1358.aspx someone asks "About extra materials... are we allowed to send in CD's of our musical performance? And if we are, when can we find sample repertoires? Should we contact the music department? Sorry to bombard you with questions, but I am just a bit confused... "

Matt responds: "YJ, send your music CD to the admissions office. We will have our music evaluators take a look."

Yeah, so the music people will tell the admissions office if it's credible and stuff...

I put my name and hometown on pieces of my app. My supplemental recs had my name and DOB. All made it to the right place.

Then again, I've got an uncommon name - if yours is "Bill Smith", I'd recommend using multiple identifiers.
