MIT Supplemental Rec Question

Hi, so I'm applying to MIT Early Action and I have a few questions about the supplemental rec my bass teacher is writing for me.

1. It will not be in until mid-november and the deadline for early action is Nov 1st. All of my required materials are already in, but is it ok that my supplemental rec arrives late?

2. For normal recs, there is a sheet that the teacher fills out with all the student information and such. Is there such a document for supplemental recs? I have looked around and have not seen anything like that. If not, how do I make sure that the rec will get sorted into my file? I'm assuming that the letter will only mention my name in the body of the rec, which would be bad for the admissions office sorting 1000+ pieces of mail/day. Suggestions?

It's okay if your supplemental letter arrives a little bit late for MIT, as they'll be sorting application pieces into folders for some time. However, you should try to ensure that the letter will get to MIT as close to the deadline as possible -- mid-November is about the latest it should arrive.

For supplemental letters, your teacher can use the normal MIT evaluation form, or he/she can write a free-form letter. Just make sure the letter has your full name and birthdate on it so it can be matched with your folder.

should I tell my teacher to include that information on his rec? Or should I have a separate sheet of paper with my information on it?

As long as it arrives with the letter, it should be fine. The easiest way to accomplish this would probably be to have your teacher write "Recommendation for Bigb14, DOB 1/1/00" in the header, but whatever works.
