Is it required/recommended (not by school but by general consensus) to apply a the school's general admissions if one is applying to its combined med program?

u must first be admitted to the general admissions. There is a second round in which adcoms decide if ur eligible for the program or not.

It's not an option. You have to apply to the school. I think the only exception (not really one) is Brown PLME. If you apply ED to PLME, you can choose if you want to be bound to Brown or just PLME. You still need to put in the full app though.

Given that you have to first complete your undergrad requirements before heading to med school, you will be interviewed/accepted first by the target school's Undergrad program director. If this individual deems you to be a good candidate then they forward your application to the affiliated med school, and you are invited for an interview at the medical school.

So, the answer is yes. You have to apply to be considered for both.

wow so i'm kinda clueless i guess

ok so how do u indicate that you are interested in that particular program? do u do it on the general admissions application or contact the program director?

It depends on the program. Some programs ask the applicant to apply to the program after they are accepted at the school. Some schools ask the applicant to apply to both simultaneously, but state that the student must first be admitted into the school before the application for the program is reviewed. Some programs will ask the student to apply to the school and indicate interest in the program (there is a place on the application or a supplement to send in) and if they deem you acceptable to apply to the program, they'll send you an app. Read the program's site...the information will be there. If it's not...call the school.
