Personal Recommendation forms? Please help! :)

Hi, i had a few questions about recommendation forms and would appreciate it if somebody would reply. I am currently applying for several boarding school and have noticed that several schools have recommendation forms for special interest like athletic or performing art instructors and ones for foreign language teachers. I have found 4 schools that have those optional forms but have yet to find a common one so they don't have to fill out several different forms. On the TABS or SSAT forms, they only have common english and math forms but nothing for foreign languge or special interest recommendation so i was wondering if there was some way to not have to make my teacher or instructor fill out several different forms for different schools?

My daughter used Gateway to Prep Schools to apply and it allowed for a third teacher of the student's choice to submit a recommendation online.

It's a shame if TABS and SSAT don't make provision for this. I know they cover a greater number of schools. It just happened that Gateway to Prep was the only online application that had all the schools my daughter wanted to apply to.

Ok, thanks so much for your help! I'm going to ask each school this question when I go for interviews.
