Online Teacher Recs Problem

Okay so how am I supposed to type my name into the downloadable forms? The first time I did it, I printed them and the forms already pre-filled in my name and everything. However, now, when I try to print the downloadable forms my name, email, high school, etc. aren't there. I really odn't wanna write my name cause it looks ugly. Is there anyway I can type my name in at all? How has everyone else been dealing with it?

type "pdf editors" in wikipedia and scroll down to the external links. if you're using a mac, there is a free one called formulate which is what I used. there are a few windows alternatives both freeware and shareware.
