Question about USC's letters of rec.

I just need some info on how I should format these and prepare them to give to my teachers. So far, I've printed out the "Counselor or Teacher Report Form"

I've filled out the Student info part in ink and written my USC ID number at the top. Is this all I have to do on this form? Other than that, should I just give it to my teacher with an envolope and they'll know what to do. Is it expected that they fill out the "Counselor or Teacher" info part and than attach there own personal letter or do they just answer the questions about me and fill out the chart?

Also, how exactly should I prepare the envelope. Is it okay to handwrite all the info on it and just attach a stamp?

Also, I haven't submitted all my application yet. So is it still okay if I have my teachers send in the letters of rec right now? I haven't gotten to a part of the online application process yet where letters of rec have even been mentioned. (I basically filled out all of Part II but haven't done the essays yet).

Yes- the instructions on the form are-
Students: Fill out Part I of this report, then have your teacher or counselor complete the rest.

Yes- this will most likely not be the first recommendation form filled out by the Counselor/Teacher. Some may have a written procedure or instructions. Just talk to them and they will tell you what else they want you to do.

We are especially interested in knowing the characteristics of this student that distinguish him or her from other candidates nationwide who are seeking college entrance. Please use a separate sheet to submit your comments.

Yes, that is fine unless your Counselor/Teacher requests a different procedure.

When you send USC your first communication (online or paper), a file will be started for you. The Counselor/Teacher Report will be placed in the file when it is received. The application will be placed in your file when it is received. It does not matter which is received first.
