USC Recommendation?

Does it have to be from either counselor or teacher? Can it be from a private music tutor or coach who knows students a lot better than most teachers? Thanks!

Most definitely!
And you can always send more than one recommendation in, so you can get one from a private music tutor or coach and also a teacher/counselor. Usually, getting one from your counselor is ideal as well as someone else.

I agree with forever09 - there is no problem with sending two or even three. Especially if the music tutor or coach can give a perspective on the applicant that won't show up elswhere in the application. It is a good idea to have at least one recommendation that is from a counselor or teacher in an academic subject.

For music tutor or coach, can they simply use free format form to provide recommendation or do they have to use the USC Counselor-Teacher form? Some of the fields on the standard recommendation form may not apply.

Use the form, but just put "n/a" or "not applicable" or in all the fields that don't apply or "see attached" across the whole academic rating area. Then attach the letter of recommendation to it. That way it is clear exactly what it is and is properly filed and considered. Be sure the USC ID and student info is filled out.
