Letters of Recommendation

So there is no actual form for the letter of recommendation (teachers) for UVa? I haven't seen any on the online application and my teachers will be sending them on paper (as opposed to e-mail). I'm asking my teachers to write their recommendations on paper and giving them to my guidance counselor for her to send out all together.

Can someone correct me or make sure I'm doing everything okay? Thanks!!

Also, do I just give my guidance counselor the "guidance counselor report" part?

I'm having two teachers give their recommendations to my guidance counselor, who will send them all out together. I will only be giving my guindance counselor literally two pieces of paper (one of which I will partially fill out). Can I just send in the supplemental part later or can I send in the supplemental part through the online application?

One more thing:
I accidentally created two different accounts (but have not paid the fee for either). I forgot my password for one of them so I tried to retrieve it but they never e-mailed me (I used to correct e-mail address), so I made another account. Is this bad?

For mine, last year, I had my counselor fill in the form. Also, two teachers to write recommendations using the school (letterhead) form. I also had recommendations from UNESCO and a lecturer at Stanford.
