Given your stats, I'd say you have a good chance (I don't know if they look at IS/OOS). Mine are actually pretty similar, give or take a few things. I'm going to assume your ECs are well written and shows commitment.
Hopefully your essays are well written and really encompasses who you are. I know a few well qualified, now UW freshmen who were denied admissions to the Honors program because their essays seemed sloppy and poorly written (they called and asked about their denial). The prompt that got them last year was something like, "If you were Amish for a day, how would you spend your day?"
The teacher recommendation should be catered towards the Honors Program (Why should they take this student?). I actually didn't know that at the time I asked my teacher for a recommendation. However, if your teacher knows how to write a great rec, you're a pretty strong candidate. This is known to be an important distinguishing factor between acceptance and denial. It seems like your teacher gave you a good rec, that's some extra points there.
I also come from a well known, highly rigorous high school in Washington. I'm not sure if that made a difference or put any weight in my GPA/types of classes I'm taking.
