Chances for a Non-Traditional Student

Hi,

I'm 27, currently enrolled at a community college, and will be receiving my Associate in Science in the spring. I'm interested in the following universities: Yale (Eli Whitney Program), Columbia School of General Studies, Johns Hopkins, and UVA. I would greatly appreciate it if someone could provide me with advice, as well as evaluate my chances for admittance into these schools. Thanks so much.

Stats:
4.0 GPA
Just completed the honors program
Took General Biology I & II w. honors
About to take Calculus with Analytical Geometry w. honors
I'm in the Honors Club
I'm a Massage Therapist, Yoga Instructor, and Reiki Practitioner.
I played in a fairly successful metal band - written up in the NY Times twice (received a full-page article one of those times), reviewed by the Financial Times, and streamed/reviewed by NPR (on their website). We also did some touring. I sing for another metal band now that's successful, but they're very extreme, so I'm not going to include them in my applications.
I provide weekly hospice care
I just applied to volunteer at a free clinic
I volunteered last year for domestic violence awareness month

My downfalls:
I did very poorly in high school. I don't even know my GPA - I just requested by transcript a week ago. I had an interesting family situation, which included taking care of my mentally ill mother alone.
I started college slowly as it had been 8 years since I'd been in high school, and I was working. I took 6 credits for too semesters, and then gradually increased. I'm taking 13 credits next semester (2 classes are sciences with labs).

Additionally, I need to take my SATs, and am currently studying for them.

I should have also included that I'm a biology major and premed. Also, I apologize for the typos. It's 2 in the morning...
