if you can pull your SAT score up to 2000, send that instead of your 28 ACT score. your courseload looks good to me (lots of APs, honors) and your GPA is very solid, so that shouldnt hurt you at all.
the other problem is that the ECs dont jump out at me. you're obviously a very sporty/athletic person, but you dont demonstrate passion in one sport. nor do you have a lot of leadership roles...
anyways, my predictions:
University of Rochester: high reach
Case Western Reserve University: low reach
Northwestern(my mom knows the dean of the math department but this is still a huge reach i know): knowing the dean miiight help, do you know him well enough to ask for a letter of rec? try and see if you can get an internship or something with him this summer, then ask him for a letter

either way, this is still a highh reach
USC: reach
University of the Pacific: no idea what this is..
Florida institute of Technology: match
