Short Additional Info section

I know it's late notice, but I am hoping a few people might volunteer to read over my short additional information section. It provides an explanation for why my grades dropped, and I'm really worried that it might come off as just trying to make excuses. I tried to make it facts only, cut and dry, and then focus on what I've learned. It is in a similar vein as a lot of probation or appeal letters I have seen here lately, and I've tried to apply the advice offered on those threads as well, but it's still different.
I would appreciate any feedback. It's a little less than 300 words, so it shouldn't take too long. Feel free to message me or post here.
