Fafsa question

For question 94a, we are asked to include amounts in W2 from boxes 12a-d, Codes D,E,F,G, and H. Our w2 has a Code C in box 12a. Should that be included or ignored?

I would not include it. Any amount in a box 12 with a code C is "Taxable cost of group-term life insurance over $50,000." It's not untaxed income, which the question is asking about, and most importantly the FAFSA instructions are not asking for the inclusion of anything designated with a code C.
