Are we allowed to send in supplementary materials such as a research paper and a mentor recommendation? Are there any formalities to be taken care of like cover sheet etc.?

You may send in supplementary materials. Recommendations should be mailed and clearly labeled as a supplementary rec. Research materials are trickier: you may just want to send an abstract, attached in the "Additional Materials" section of the Common App.
