Writing a letter/email to a prof. for internship?

I'm planning on interning with a college professor this summer. So I was wondering how I should write a letter to them? Can someone write me up a 'outline' of what to say, and the tone of voice? Also, if it matters, the professor and the college he's teaching at is in another country (he's English speaking).

Should I seem kind of conceited about myself? Should I attach a resume right with the original email? Would this be a good time to bring up accomondation issues, etc.?
Anything else to keep in mind?
