  Is this the one where you fill out a bunch of info so they can match you up with someone else? If so, where do you get this form?

  yeah, I didn;t get one either. I got another app through e-mail for Parkside.

  I didn't get one either, I live in NH, so anything that gets mailed separately takes forever to get here. I've already sent in my housing form and my commitment to register, so I think I should be getting one. We shall see.

  Aren't you supposed to get the survey after you turn in the housing form? I don't think it's supposed to come with the admittance letter.
