<p>1) Yes, just use a blank form and write your USC ID neatly. The stickers have, in the past, been sent after you fill out the Part 1 application, but many students don’t get them. You don’t really need them. If they arrive before you send your materials in, just stick one on each document or envelope over the place you wrote in your ID number.</p>

<p>3) This happened to my D, and she called to ask. They said she could use either number, so she chose the one she received when she submitted Part 1. The most important thing is to chose ONE and stick with it - be sure to double-check everything you send and receive to see that the number you decided upon is there. </p>
