The following plans provide full coverage for care provided by Cedars-Sinai Medical Center and its physicians:</p>
<p>Health Net Platinum PPO
Health Net Gold PPO
Health Net Silver PPO
Health Net Bronze PPO Plan (available to all) - also available on Covered California
Health Net Catastrophic PPO Plan (available to people under the age of 30) - also available on Covered California
Health Net will not be grandfathering any individual members.</p>

<p>Aetna
If you have a private policy for yourself or your family, you will need to find a new insurance provider. As of Jan. 1, 2014, Aetna will not be offering plans to individuals in California.</p>
<p>The one exception is Aetna’s Costco Personal Health Insurance program.</p>
<p>If you have the Costco plan and renew it prior to Dec. 15, 2013, it will provide you with an additional 12 months of coverage from the renewal date – including coverage for care provided by Cedars-Sinai.
If you are a Costco member and not currently covered by Costco Personal Health Insurance, you can still purchase individual coverage. Your application must be submitted and you must be enrolled by Dec. 15, 2013. The coverage, if approved, will be for 12 months.

<p>Cigna
Cigna is changing its provider network for individual coverage.</p>
<p>If you submit your application and are enrolled in one of their full-network plans (called OAP Network Plans) by Dec. 15, 2013, you will have coverage for care from Cedars-Sinai through the end of 2014. (Note: Applications for these plans require medical information and approval is subject to medical underwriting.) All current Cigna individual and family plan members who are enrolled in OAP Network Plans will be able to keep full coverage for care from Cedars-Sinai until the end of 2014.
After Dec. 15, 2013, Cigna will only offer narrow-network plans (called Local Plus plans) as an option for individuals and families in California who are new enrollees. These plans do not provide full coverage for care from Cedars-Sinai.

<p>Anthem Blue Cross
Anthem Blue Cross’ new individual and family plans for 2014 will exclude the majority of Cedars-Sinai physicians from its new narrow network of providers.</p>
<p>If you currently have an individual or family plan from Anthem Blue Cross that includes Cedars-Sinai, you may be able to keep that plan through a process called grandfathering.</p>
<p>Please call the member services phone number on the back of your insurance card to find out if you are eligible. If so, all you need to do is continue to pay your premiums and you automatically will keep your current plan.
Anthem Blue Cross will not allow grandfathering if your coverage started after March 22, 2010. If your plan began after that date, you will need to change plans before Jan. 1, 2014.
