<p>and second, here is what i suggest. It doesn’t matter if you mail in the ED contract form yourself, or if your counselor does – the Operations staff will process it the same way. The important thing is that you, a parent, and your counselor all sign the form. (When I applied there was a line for me to sign as the applicant – if there isn’t, then I would just add your signature somewhere on the contract page.)</p>
<p>The other important thing is that when the ED contract form is set-in you include your name, birthdate, and your address so that the Operations staff can match it to your online application. (SSN are no longer being used).</p>
