<p>You are free to submit more than one recommendation, and it seems most applicants who post on College Confidential submit two or three. It might be confusing to have a sort of half-and-half recommendation, with neither of them complete, so I don’t recommend having one person fill out the front of the form and another do the attached letter.</p>
<p>My suggestion would be to give your couselor the form to fill out and allow them to go ahead and attach their own recommendation (assuming you feel they will write a positive rec), and the give the form to your teacher and just write a line through the section more directed at a counselor and write “N/A.” Have them fill out their identifying info and just put it in the envelope with their letter. The form will assist in matching it up with your application.</p>
<p>USC is VERY flexible about this, so it would also be fine to send in an additional teacher recommendation without the official form - just the letter. Be sure your full name as it appears on your application is on the letter, and either your USC ID# or your birthdate so they can match it up.</p>
