<p>The Student Health Center and NYS Law require newly admitted students to submit the health form to be signed by a physician, documentation of 2 MMR vaccines, and documentation of Meningitis vaccination. The docs and health form (sent out to all newly admitted students) have a deadline of 8/15/11 for submission if you are a Fall admit.</p>
