Residents get paid according to their PGY year, not what specialty they are in.
Neurosurgeons don’t get paid more than family practice residents, even though they work more hours.

Internal medicine isn’t an easy residency. Medicine residents have overnight call THROUGHOUT residency, usually q4 (every fourth night) unless you’re one of the rare lucky ones at a q5 residency program.

Subspecialties usually graduate to a home-call system after a few years of residency. I’m a neurology resident (PGY-4) and I did in-house call during my PGY-1 and PGY-2 years, and after that it’s q3 home call. I work around 50 hours/wk in the hospital. My friends in internal medicine are still working 80 hrs/wk (all 3 years). I don’t know any surgeons who work 80 hrs/wk. It just isn’t possible. I suppose some surgery programs must abide by ACGME work restrictions, but I bet most of them don’t.
