I don't have an ID, how to prepare evaluation forms? (rushing for dec 1 deadline)

<p>I don’t know how to get my teacher/counselor form ready to give to my teacher/counselor.</p>

<p>I submitted Part I several weeks ago but I never got a USC ID number. Also, a week ago, I randomly got an email saying that my paper application was in the mail. It hasn’t arrived yet…</p>

<p>Since I have no paper application, nor a USC ID number (for writing on teacher/counselor forms), how do I get the evaluation forms ready so that I can give it to them with an envelope and everything? USC doesn’t seem to have an online mechanism for doing these teacher/counselor forms (unlike Common App and Columbia App)… and I want to apply by the December 1st deadline for scholarships.</p>

<p>I realize we need to give our teachers/counselors enough time. Since my deadline is December 1st, I need to figure out how to do this soon. I want to give my teacher/counselor the form sometime this week. I don’t know if my ID and/or paper application will arrive in the mail soon enough…</p>

<p>PS: I noticed that by finishing my online application, USC will prepare the teacher/counselor forms for me online, ready to print. However, I don’t want to rush on finishing this application, just to have the forms for teacher/counselor…</p>

<p>I think just your application has to be in by Dec 1, not your evaluation forms.

<p>should I just print out the forms and give 1 to my counselor and 1 to my teacher without putting the sticker or the ID code?</p>

<p>or should I wait until USC gives it (one or the other) to me? I could email them and wait, or print the forms and just give it to my teacher/counselor. what do you guys think I should do?</p>

<p>Others here have stated that admissions told them you can write your Social Security Number or birthdate and it will get correctly filed.</p>

<p>But I would call USC Monday morning to get the ID, write it in on the forms then give it to the counselor and teacher.</p>

<p>ok i’ll call them on monday. my teacher+counselor will have enough time, even if I give it to them later this coming week, like friday.</p>
