Insurance Company Home Visit?

Having been through unfortunately more than a couple of discharges from the hospital with my almost 87 year old mom, having someone come to the home or assisted living after discharge is quite common and expected (visiting nurse assessments, PT,OT). Sounds like you are very involved at this point so being present at hospital discharge to hear the discharge plan may help with communication issues (or following up by phone if you can’t be there in person). Sounds like you already have permissions in place to talk with medical people, but if not, would be a good time to put all of that in place. Good luck! These kinds of things are tough.
