I was trying to understand the 3 items you mentioned and could not get a clear understanding of what is the difference and where to obtain the forms. Google search is coming up with so many variations of forms and not able to decide is Proxy and Directive is one and the same.
Looked at this site to see the definition and but still not clear.

Proxy form has so much variation. The form in OU Medicine site is simple and just a page. Not sure if that form is sufficient or need to have all the 3 forms.

ok gov site form says directive but with in that it has living will also. Also it expects to sign these doc in OK county office.

For OK HIPPA form, at least got a link from OK Gov. Hope all hospitals or MOB accepts this std form.
