<p>Last year my D gave the GC and teachers the forms before she completed Part II. The HS required the students to submit the forms quite early so that GC/teachers had plenty of time. She downloaded the forms from the USC site somewhere if I remember correctly. They therefore did not have the ID # since the forms were not linked to her app, so she added that in pen. Did you do Part 1?, …if so you must have your ID # by now. If not, then indicate your name, DOB, and high school on the forms. I hope you can find them. Unless something has changed this year, they were downloadable in the Admissions section of the USC site.
If you can’t find them, call USC. They are very flexible and helpful with any problems or concerns.</p>
