A woman can bleed out in minutes during a “normal” vaginal delivery too.

And a probably not well-known fact-- you don’t need to be a OB/GYN to perform a C-section. A family medicine physician who completes a 1 year fellowship in “women’s health care” is also licensed to perform C-sections, as well other other routine gynecological and obstetric procedures such a D&Es, D&Cs, and inserting IUDs.

I’d be willing to bet money these rural birthing centers are not staffed by Ob/Gyns but by FM doctors. D22 has a couple of friends who did a women’s health fellowship and both now are FM docs at rural clinics (Moab, UT and Cortez, CO) where their practices include doing C-sections.

My friend (from my NICU days) had twins 2 years after our 24 weekers were born. One of the nurses (and a good friend of hers) came and took one twin saying “She looks jaundice” and off they went. Two days later everyone released. My friend was so funny when she said “they expect you to just take the babies HOME.” Her first had stayed 105 days.

Having seen many emergencies like this, with both mothers and babies, at this point I feel as if the only safe birthing center is the one that is physically on the same floor as the OR and the NICU. Let it be as warm and fuzzy and homey as possible, have a separate entrance, feel like a beautiful welcoming inn. But on the same floor, through double wide doors, the OR and the NICU!

Of course it would be much cheaper, safer and all-around healthier for Medicaid to pay for it all than to pretend that there’s a safe solution close to home.

But sometimes there are alternatives like a Ronald McDonald house or similar. It may seem cheaper for the family, but I really hope the birthing centers or place where the out-of-hosp cesareans are being performs (non-emergency) are doing everything they can to NOT have the procedure out of hospital. I hope they are explaining the risks and helping the family overcome the obstacles.
