<p>The 2012 candidate information booklet that was in the PTR lists nine documents that must be returned in the manila envelope. The SF-86 was one of them. It would be a good idea to read the information booklet to know what they are asking of you and it has a number of other things that are required. i.e. immunization info, glasses, I-Day info etc.</p>
