<p>My son’s packet was also missing the SF-86 – he was able to download it from the CIS site and print it out. Unfortunately, my husband had already mailed the rest of the materials that were to go in the postage-paid envelope (he was only supposed to make copies!), so my son called admissions for guidance. He was told to mail the form to the same address: U S Naval Academy, Commandant of Midshipmen, 101 Buchanan Road RM 4001, Annapolis MD 21402-9904. So it was just sent separately.</p>
