I can’t speak for the others, but with Cal Poly projecting a 12.5% admission rate for Poly Sci and the OPs capped and weighted GPA being in the 25th percentile for the College of Liberal Arts, I would consider it to be a reach.

There is one caveat. Their admissions algorithm, at least the last time it was public facing, included a boost for attending what used to be called a Hayden Partner School. They are, as you described, low income public high schools. With that adder in the algorithm, it could bring it to a match.

I agree with others, UCs are difficult to determine. Looking at your stats, I would say if you applied to UCR that should be likely, UCSC is also a likely target, UCD might be a target, UCSD is too difficult to tell, maybe a target to reach, the rest are reaches. I presume since you indicate BYU is guaranteed that you are part of the LDS?
