<p>Its a double sided form thats meant to be sent along with the the School Report - i.e. its a ‘Supplement’ to the School Report. Stupid huh? Its just a bunch of more evaluations for your counselor to fill in, along with his address & details (again!) and all your standardized test scores. Utterly pointless…But thats just me.</p>
