PTR - Immunization Record Form

Yes.
Having said that, I would:
-bring a copy of your updated vaccination record with you in YOUR pocket (not left with your parents)
-HIGHLIGHT any updated vaccination information directly on the sheet.

As good as they are, do not count on the medical staff to reconcile what has “changed” on your list… they will be processing over 1000 of you on I-Day… so make it easy for them to review your needs.

They should arrive back to back. As they are sent out after you resond, send the acceptance card in as soon as you get it.

You can fax any last minute paperwork to the admissions dept if necessary (call them first). Have your soon-to-be-anointed-plebe bring a copy of it with them as well.
The candidate number is all you need to access CIS. It is my understanding additional information is posted there once your son/daughter accepts the appointment by returning the card… it takes a few days after that, but generally gets updated quickly.

There is still plenty of time to get this all done; even the rare candidates with an offer of appointment in the days just prior to I-Day somehow manage to get it done!

Keep the faith!
