<p>Yes, there is a Transcript Request Form (Form 7), but you don’t have to use it. USC provides it as a convenience if you need it. It is available in post #8 of the New Applicant Toolkit thread I linked. You can use that to sign and submit to your registrar to ask them to send an official transcript to USC, but you don’t have to.</p>
<p>You can use you school’s request form, USC’s request form, you can mail the sealed transcript with your recommendation(s), or mail it by itself, or your school can mail it or submit it electronically. As long as an official transcript makes its way to USC, they are very flexible about how you get it there.</p>
<p>Your **midyear report **is as I described it in post #2: You enter your midyear grades online and then mail or fax a copy of your report card. No transcript is required.</p>
