Many pickups, large SUVs, and vans have the spare tire mounted externally under the rear of the vehicle. There is a long rod provided to insert into a small hole in the rear bumper area to lower the spare tire.

See page 204-205 of http://www.fordservicecontent.com/Ford_Content/catalog/owner_guides/04f12og5e.pdf (2004 Ford F-150 owner’s manual) for how the spare tire is lowered from its storage place. The next few pages contain the instructions for installing the spare tire.

The owner’s manual linked above says that the wheel nut torque for the 2004 Ford F-150 is 150 lb-ft, which is higher than for most cars. If the lug wrench is only about a foot long, you need to be able to deadlift 150 pounds, or weigh at least 150 pounds and stand on the end of the lug wrench, to loosen the nuts.

The wheel and tire for a full size pickup can be quite heavy as well.
