It’s too soon to tell if you will have the stats needed to get into ANY med school, much less a top 25 med school.
Frankly, the D in ochem is very bad sign and earning a GPA and sGPA> 3.8 isn’t going to be as easy as you seem to believe.
BTW, it take much more than merely great numbers to get accepted into top med school. You need great ECs to go along with the great stats. Your numbers only get you considered for admission, it’s your ECs (and PS, and essays and LOEs) that get you invited to interview.
