A Frequent Mover needs help Applying

Currently I attend a prestigious school in my area; however, I attended two other public schools before going to my current school (because of moves). I have a 3.5 GPA (the average of my GPAs from all of my schools). I have been extremely involved in all of my schools. While being a 3 sport athlete all three years, I was captain of my lacrosse team freshman year, sophomore year I was involved in model UN, junior year I was athlete of the year, Vice-president of the MUN club, I received the PVSA, I was a sports council representative, a member of the National Art Honors Society, and placed in the pool for national merit scholars. However, my test scores are not the best. I got a 36 on the reading portion of the ACT but my overall ACT score is a 28. I also have a 1890 SAT score. I also received a 4 on both the AP Euro and World exams, and a 5 on the AP English Language exam.
I just really am stressing out about finding a college because it is rather difficult to compare myself with the usual GPA and test scores due to my moves. Do my extracurricular activities set me apart? Does my continued persistence help?
I do not want the Ivy league experience, I want the small, relatively well known liberal arts college. Do I have a chance? Obviously I should attempt to raise my test scores, but what else should I try to do? Any colleges that look beyond the test scores but that are respectable?
Thanks!
