A little help at a second chance.

Last year I did very poorly, and I am now trying to improve my situation. I am currently attending a community college, and wish to transfer to a state school. I feel like I might be doing too little, too late and want to know what I can do to reach my goals of transferring. The following is my situation in more detail.

Last school year, fall 2008 to spring 2009, I was attending a private school, Polytechnic University of NYU. At the end of the year I was academically disqualified, receiveing only 4 credits the entire year. I am now attending a community college in a program aimed at getting students back on their feet.
I want to transfer out of New York City, and have been looking at several SUNY’s. I did pretty well on the SAT’s, receiving 720 on the math and 640 on the reading. I give great interviews and I have received awards from congressman for my community service. I wish to pursue a degree in civil engineering.
What do I have to do in order to be accepted to a school like Stony Brook or any SUNY that has a decent engineering program?
