In books and web sites when a school’s average GPA is listed as a 3.5 does that usually mean W or UW and is it based only on academics or includes all courses even electives?

If a school gives a 4.0 for an A in an acedemic/regular class and that same A gets a 4.5 in Honors and a 5.0 in AP --how would that system get recalculated? For example with a:

3.5 overall GPA based on 3 Honors classes where 3 Bs were equal to 3.5 and all other non Honors courses that same B is equal to 3.0, how would that be recalculated, assuming the Honors courses were core academics and the non-Honors were core academics plus electives like languages, PE, Health, Photography?
