Academic dismissal reinstatement letter review

I am asking for your help in reviewing my reinstatement letter after being academically dismissed. Any feedback would be greatly appreciated. Thank you!

Dear Dean xxxx,
I am writing this letter to request reinstatement from xxxx after being dismissed for not meeting the Satisfactory Academic Progress requirements. I was embarrassed and disappointed at myself after receiving the letter, but I can’t say I was surprised. My lack of focus and poor time management skills have led me to this point and I have no one to blame but myself.

As a Center for Distance Learning student I know that organization and time management are the most important keys to success, but I let that get away from me. I had poor time management skills and was not able to focus on my studies when I was home. I found myself putting assignments off because I knew they weren't due until the end of the week, leaving me with an overwhelming amount to do in the last two days. Instead of learning from my mistake I continued to put off work in hopes of getting to it at a later date. This brought me into a cycle of constantly trying to catch up and stay with the class at the same time, which was a recipe for failure. By the time I reached out to my professors it was already too late and there was nothing I could do to make up the late work. I also never took advantage of the available support that xxxx offers, like workshops to help CDL students work on time management, stress, and writing skills.    

  I realize now that I need to take a proactive approach when it comes to my studies. I plan on doing this in a number of ways, all of which will make me more organized and put me on the path to success. The first thing I plan to do is create a study schedule and set specific time frames for each class. This will break my habit of waiting until the end of the week to try and get everything done. I know that I easily get distracted at home so I'am going to get into a routine of going to the library Monday through Friday. I will print out the course schedule of each class so I know the due dates for all assignments. This will help me budget my time to determine which classes need attention. 

As a student at the Center for Distance Learning I felt that I needed to be independent and do everything on my own. I now realize this is not the right approach. There are plenty of resources I will take advantage of at the xxxx learning center which include webinars, the writing center, and library. I will also contact my professors and adviser if I feel I need any additional help. Asking for help when I need it has been a challenge for me, and I will not let that happen again. 

In moving forward and using the actions I have described I will complete four courses this September term and the four remaining courses in the spring, allowing me to complete my degree in May of 2016. Earning a bachelors degree has always been a goal of mine, as I would be the first person in my family to earn such an honor. I think your letter is too wordy. The intro paragraph is OK. The next two have a lot of extra, and unnecessary words.

Try to eliminate repetitive wording. Just say what happened, then list out what you have already done. Then list out what you plan to do.
